  • Secondary ID: birth certificate, passport or Social Security card (front and back)
  • High school or college diploma or transcript

Once your application is submitted, pay the application fee through our secure Square checkout to complete your submission.

Please allow up to 72 hours for review. We will follow up by phone or email once it has been processed.
